DescriptionFor the representation of a follow-up after an intervention.
UseThis archetype is used to document a follow-up of a patient after an intervention. This archetype is used to document information on how contact is established with the patient. The result of the follow-up is documented by integrating various CLUSTER archetypes (e.g. CLUSTER.hospitalization, CLUSTER.death_details). Possible types of contact are telephone calls, direct communication (e.g. when the patient is presented in hospital again) or a printed/machine letter (e.g. doctor's letter). In case no contact can be established with the patient, other persons can be specified as a source of information, e.g. a relative or a doctor.
